Job Summary
The Admissions Nurse is responsible for facilitating the admission process of new residents to our Skilled Nursing Facility (SNF). This role involves assessing potential residents, coordinating with healthcare professionals and families, and ensuring a smooth transition into the facility. The Admissions Nurse plays a vital role in gathering essential information and supporting residents and their families during the admission process.
Key Responsibilities
- Conduct initial assessments of potential residents to determine their eligibility and appropriateness for admission to our SNF.
- Coordinate with hospital discharge planners, physicians, and other healthcare professionals to gather necessary medical information and ensure a seamless transition from the hospital to our SNF.
- Communicate with residents and their families to explain the admission process, address concerns, and provide information about the facility's services and policies.
- Collect and review medical records, including physician orders, advance directives, and medication lists, to ensure comprehensive care planning and adherence to regulatory requirements.
- Collaborate with the interdisciplinary team, including nurses, therapists, and social workers, to develop individualized care plans based on the residents' needs and goals.
- Coordinate with the business office to verify insurance coverage and obtain necessary authorizations for admission.
- Arrange for transportation of residents from hospitals or other facilities to our SNF, ensuring a safe and comfortable transfer.
- Conduct thorough assessments of residents upon admission, including physical, psychological, and social evaluations.
- Provide orientation to new residents and their families, familiarizing them with the facility's policies, services, and routines.
- Ensure accurate and timely documentation of admission assessments, care plans, and other required paperwork.
- Assist with coordinating and scheduling ancillary services, such as laboratory tests, radiology procedures, and specialist consultations.
- Serve as a liaison between residents, families, and the interdisciplinary team, advocating for residents' needs and preferences.
- Collaborate with the nursing team to ensure a smooth handover of care and effective communication during the admission process.
- Stay updated with the latest regulations, guidelines, and best practices related to SNF admissions and maintain compliance with legal and regulatory requirements.
- Participate in facility tours, marketing events, and community outreach initiatives to promote our facility’s services and establish positive relationships with referral sources.
Qualifications
Qualifications and Skills
- Current and valid nursing license (Registered Nurse or Licensed Practical Nurse) in the state of Alabama.
- Previous experience in a long-term care or skilled nursing facility, preferably in an admissions or care coordination role, is preferred.
- Sound knowledge of medical terminology, healthcare procedures, and regulatory guidelines related to SNF admissions.
- Strong assessment and critical thinking skills to evaluate potential residents and identify their care needs.
- Excellent communication and interpersonal skills to effectively interact with residents, families, and healthcare professionals.
- Ability to handle sensitive and confidential information with discretion.
- Proficiency in documentation and record-keeping.
- Detail-oriented and organized, with the ability to manage multiple tasks and priorities.
- Familiarity with insurance verification and authorization processes.
- Knowledge of Medicare, Medicaid, and managed care regulations is advantageous.
- Compassionate and patient-centered approach towards resident care.
Working Conditions
- Works in office area(s), warehouse, pharmacy, and some positions work in a long-term care environment (i.e. nursing home, drug rooms, nurses’ stations resident rooms, etc.).
- Moves intermittently during working hours.
- Is subject to frequent interruptions.
- Is involved with customers, and customer representatives, family members, residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
- Is subject to hostile and emotionally upset residents, family members, personnel, and visitors.
- Communicate with the designated facility staff, nursing personnel, residents, or family members.
- Works beyond normal working hours and on weekends and holidays when necessary.
- Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
- Is involved in community/civic health matters/projects as appropriate.
- Attends and participates in continuing educational programs.
- Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.
- Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS, COVID-19, and Hepatitis B viruses.
- May be subject to the handling of and exposure to hazardous chemicals.
